Please note that our clinic does not have psychiatrists; therefore, we cannot issue medical certificates for sick leave due to psychiatric disorders. For sick leave due to injury, if a definitive diagnosis cannot be made, a certificate may not be issued.

The certificate includes your medical history, current medications, and present condition. Please bring documents that show your medical history and any medications you are currently taking.
Depending on the content, immediate issuance may not be possible. For English certificates, please allow about one week, so plan your visit with sufficient time.
When traveling abroad, English medical certificates can be useful in various situations. If you have a chronic condition or need to bring regular medications, having an English certificate can be convenient at immigration.
English certificates may include vaccination certificates, medication carriage certificates, and health check certificates. For visa applications, schools abroad, overseas assignments, and in some states, an English medical certificate may be required upon entry. Please confirm in advance.
This certificate is issued when needed for applications to receive life insurance benefits following treatment. Please bring any documents that can verify the course of the illness or injury.
For infectious diseases such as influenza, pertussis, measles, mumps, rubella, chickenpox, or pharyngoconjunctival fever (adenovirus), a certificate of recovery may be required by schools or workplaces after a suspension of attendance.
In principle, it is preferable to obtain this certificate from the medical institution where you were diagnosed. However, if your primary clinic is closed and cannot provide the document, we may issue it as long as the course of symptoms, date of diagnosis, and diagnosis name can be confirmed.

Medical certificates can be collected with the consent of the individual or their authorized representative. The individual must be examined in person for the certificate to be issued.
We apologize, but we cannot send the medical certificate by mail. If you bring a consent form, someone else may collect it on your behalf.
When taking a leave of absence due to illness or injury, a medical certificate from your primary doctor is required to confirm that the leave is necessary. Similarly, when returning to work, a medical certificate is needed to confirm whether you are in a condition to resume work. Once the return-to-work certificate is submitted, the company can start the formal process for your return.
If the medical examination was conducted at our clinic, we can issue the return-to-work certificate as well. However, if your leave-of-absence certificate was issued by another medical institution, it is recommended to have the return-to-work evaluation done there. Since we have not been monitoring your condition or progress, it is difficult for us to determine whether your health is stable. This could make it unclear whether you are ready to return to work and whether any workplace accommodations are necessary.
At our clinic, we issue medical certificates only when the submission destination and purpose of use are specified. The reason is that a medical certificate issued by a medical institution is considered an official document, and we have a responsibility to manage it properly.
