1. The Right to Receive Quality Medical Care Fairly
Patients have the right to receive safe and appropriate medical care without discrimination.
2. The Right to Freely Choose Doctors and Hospitals
Patients have the right to freely choose their doctors and hospitals when undergoing examinations or treatments, and to seek a second opinion (the opinion of another doctor).
3. The Right to Sufficient Explanation and to Decide Treatment by Their Own Will
Patients have the right to receive explanations about treatment information and to freely decide whether or not to undergo treatment.
If a patient is unconscious or unable to make decisions, a legally designated representative has the right to make decisions on their behalf.
4. The Right to Know Their Own Medical Information
Patients have the right to receive sufficient explanations about their medical information in a way they can understand.
5. The Right to Protection of Personal Information
Patients have the right to confidentiality of all personal information that could identify them, including health status, diagnoses, and treatments.
6. The Right to Health Education
Patients have the right to receive education on personal health and healthcare services, including disease prevention and early detection.
7. The Right to Dignity
Patients have the right to have their personality and values respected, and their dignity as human beings preserved.
8. The Right to Religious Consideration
Patients have the right to consideration for their religious beliefs, if such consideration is necessary.
1. Providing Information
To ensure safe medical care, please provide as accurate information as possible about your body and illness.
If the patient is a minor or unable to communicate, family members or accompanying persons are requested to provide the information on their behalf.
2. Cooperation in Medical Care
As a member of the medical team, patients have both the right and responsibility to participate in their treatment.
If you have concerns or questions about examinations or treatments, please feel free to ask until you are fully satisfied.
3. Preventing Patient Misidentification and Safety Measures
We ask for your cooperation in efforts to prevent misidentification and fall accidents. Before procedures or consultations, we will confirm your name and date of birth to verify your identity. To prevent falls, staff may ask for your cooperation in safety measures. If you have experienced a fall within the past year or need assistance, please inform our staff.
4. Following Medical Instructions
In order to carry out examinations and treatments safely and effectively, we ask for your cooperation in adhering to agreed treatment plans and physicians’ instructions after proper explanation and consent.
5. Infection Prevention
We ask for your cooperation with infection prevention measures to ensure a safe environment for all patients and staff.
6. Smoking Prohibition
Smoking is strictly prohibited inside the clinic. Thank you for your understanding and cooperation.
7. Violence, Verbal Abuse, and Disruptive Behavior
Acts such as violence, abusive language, harassment, or any behavior that disturbs other patients may result in refusal of medical services depending on the situation.

At our clinic, we distinguish between items for which explanations and consent are confirmed verbally (general consent) and items for which formal written explanations and consent are required (individual consent).
For general consent items, since many of them do not require the presence of a physician, they are handled through verbal explanation and confirmation of consent. If you have any questions, please do not hesitate to ask a doctor or nurse.
[General Items]
Medical interviews, visual examinations, physical examinations, body temperature measurement, body measurements, blood pressure measurement, and photographing affected areas with cameras, etc.
[Tests / Monitoring]
Blood tests, immunological tests, infectious disease tests, urine and stool tests, microbiological tests, percutaneous biopsy (e.g., for corns), rapid tests such as various antigen tests, physiological tests such as electrocardiograms and ultrasound, breath tests, ophthalmological tests, hearing tests, general X-ray imaging, monitoring such as blood pressure, pulse, and oxygen saturation, and intradermal tests such as tuberculin reactions.
[Procedures]
Venous blood sampling, wound care (wound cleaning, suture removal, suturing, incision, abscess drainage, foreign body removal, etc.), nail clipping, shaving.
[Medication / Administration]
Routine medication (such as inhalation), injections, insertion of peripheral intravenous catheters (securing IV lines), oxygen administration.
The above medical procedures are performed by personnel with sufficient experience; however, there may still be rare and unexpected complications such as bleeding or anaphylaxis. In such cases, treatment of complications will be covered under regular health insurance. We kindly ask for your understanding in advance.
[Multilingual Interpretation]
Our clinic accepts foreign patients and provides medical guidance in multiple languages as well as culturally and religiously sensitive care. However, when interpretation is provided by family members or associates of foreign patients, please understand that there is a risk of misinterpretation or ethical issues.
[Religion / Culture]
Our clinic respects patients’ religions and cultures, but we may not always be able to accommodate requests regarding the gender of healthcare providers.
[Provision of Information to Pharmacies, etc.]
For the purpose of enhancing the safety of pharmacotherapy and ensuring smooth coordination, we may provide necessary information such as patients’ medication history, history of adverse reactions, clinical test results, and explanations about medications to dispensing pharmacies or other medical institutions.
[Privacy]
Photography or video recording within the clinic using cameras, smartphones, or video devices is prohibited, as it may infringe on the privacy of others.
[Jurisdiction]
Any litigation arising from medical services at our clinic, including issues related to interpretation, translation, or administrative procedures, shall fall under the exclusive jurisdiction of the Tokyo Summary Court or the Tokyo District Court as the court of first instance.
[Governing Law]
Consent at our clinic shall be governed and interpreted in accordance with Japanese law.
[Security]
For security within the clinic, consultation rooms may be monitored and recorded (audio and video). Recorded data will be retained for a period specified by our internal regulations.
