Under the principles of the World Medical Association(WMA), “Declaration of Lisbon on the Rights of the Patient,” our clinic is committed to providing medical care that respects patients’ human rights. On the other hand, in order to uphold these rights, it is crucial that patients take initiative and participate in their own care. We kindly ask that you read and fully understand the patients’ rights, as well as requests from us as below.
1. The Right to Equally Receive Medical Care of Good Quality
The patients are entitled to receive appropriate medical care without discrimination.
2. The Right to Freely Choose Doctors and Medical Facilities
Patients are entitled to freely choose and change their physician or hospital for any examination or treatment. They also have the rights to ask the opinion of another physician at any stage.
3. The Right to Receive Sufficient Explanations and Determine Their Treatment Methods Based on Their Own Judgments
Patients have the right to receive detailed information about treatments and to make their own decisions freely.
If a patient is unconscious or unable to decide, a legal representative has the right to make decisions on his/her behalf.
4. The Right to Know Their Own Medical Information
Patients have the right to receive sufficient explanations about their treatment in a manner that they can understand.
5. The Right to Confidentiality
All identifiable personal information such as a patient’s health status, diagnosis and treatment must be kept protected.
6. The Right to Health Education
Patients have the right to receive health education on personal health and health services such as prevention and early detection of illness.
7. The Right to Dignity
Patients have the right to be respected regarding their personalities and values and be protected with dignity.
8. The Right to Religious Consideration
Patients have the right to receive special consideration if they need it based on their religious beliefs.
1. Regarding Information Provision
In order to provide medical care safely, please answer as accurately as possible about your medical condition and history. If you are a minor or unable to communicate by yourself, a family member or accompanying person should relay your symptoms on your behalf.
2. Regarding Cooperation with Medical Care
Patients have the right and responsibility to participate as members of the medical team in their treatment. If you have any concerns or questions about the medical tests or treatments you will undergo, please feel free to ask until you are fully convinced.
3. Regarding Measures to Prevent Patient Misidentification and Safety Measures
We ask for your cooperation in our efforts to prevent patient misidentification and falls. Before any procedures and consultations,If you have a history of falls within the past year or require assistance with mobility, please inform us.
4. Regarding Instructions from Healthcare Providers
In order to ensure that tests and treatments are carried out safely and effectively, we ask for your cooperation in adhering to the treatment plan and instructions that have been explained and consented upon.
5. Regarding Infection Prevention
We kindly ask for your cooperation with our infection prevention measures within our clinic. Upon arrival, please disinfect your hands using the alcohol-based sanitizer and comply to proper cough etiquette. Depending on the patient conditions or the prevalence of infectious diseases, our staff members may ask you to wear a mask or wait outside the clinic. We appreciate your understanding and cooperation.
6. Regarding No Smoking
Smoking is prohibited throughout the clinic. We appreciate your understanding and cooperation.
7. Regarding Violence, Verbal Abuse, and Harassment
If any violent behavior, verbal abuse, harassment, or other action that may disturb other patients occurs (or is observed), we may refuse to provide medical examination depending on the situations.
8. Regarding Guardians Bringing Their Children
We have hazards such as medical equipments and infectious materials within our clinic. Please keep a close watch on your children at all times. If you need any assistance, please ask our staff members.

At our clinic, we distinguish between items for which explanations and consent are confirmed verbally (general consent) and items for which formal written explanations and consent are required (individual consent).
For general consent items, since many of them do not require the presence of a physician, they are handled through verbal explanation and confirmation of consent. If you have any questions, please do not hesitate to ask a doctor or nurse.
[General Items]
Medical interviews, visual examinations, physical examinations, body temperature measurement, body measurements, blood pressure measurement, and photographing affected areas with cameras, etc.
[Tests / Monitoring]
Blood tests, immunological tests, infectious disease tests, urine and stool tests, microbiological tests, percutaneous biopsy (e.g., for corns), rapid tests such as various antigen tests, physiological tests such as electrocardiograms and ultrasound, breath tests, ophthalmological tests, hearing tests, general X-ray imaging, monitoring such as blood pressure, pulse, and oxygen saturation, and intradermal tests such as tuberculin reactions.
[Procedures]
Venous blood sampling, wound care (wound cleaning, suture removal, suturing, incision, abscess drainage, foreign body removal, etc.), nail clipping, shaving.
[Medication / Administration]
Routine medication (such as inhalation), injections, insertion of peripheral intravenous catheters (securing IV lines), oxygen administration.
The above medical procedures are performed by personnel with sufficient experience; however, there may still be rare and unexpected complications such as bleeding or anaphylaxis. In such cases, treatment of complications will be covered under regular health insurance. We kindly ask for your understanding in advance.
[Multilingual Interpretation]
Our clinic accepts foreign patients and provides medical guidance in multiple languages as well as culturally and religiously sensitive care. However, when interpretation is provided by family members or associates of foreign patients, please understand that there is a risk of misinterpretation or ethical issues.
[Religion / Culture]
Our clinic respects patients’ religions and cultures, but we may not always be able to accommodate requests regarding the gender of healthcare providers.
[Provision of Information to Pharmacies, etc.]
For the purpose of enhancing the safety of pharmacotherapy and ensuring smooth coordination, we may provide necessary information such as patients’ medication history, history of adverse reactions, clinical test results, and explanations about medications to dispensing pharmacies or other medical institutions.
[Privacy]
Photography or video recording within the clinic using cameras, smartphones, or video devices is prohibited, as it may infringe on the privacy of others.
[Jurisdiction]
Any litigation arising from medical services at our clinic, including issues related to interpretation, translation, or administrative procedures, shall fall under the exclusive jurisdiction of the Tokyo Summary Court or the Tokyo District Court as the court of first instance.
[Governing Law]
Consent at our clinic shall be governed and interpreted in accordance with Japanese law.
[Security]
For security within the clinic, consultation rooms may be monitored and recorded (audio and video). Recorded data will be retained for a period specified by our internal regulations.
